Summary

 
The Team Lead-Care Management role coordinates daily activities of care coordination colleagues and advises on priority projects by consulting with manager. Provides general administrative direction over patient education related to illness and planned treatment, medication management in collaboration with the primary care provider, and promotes patient self-management. Works in partnership with entire healthcare team to manage transitions of care and close gaps in care. Provides oversight in standardizing processes and procedures related to patient care and medical management.

Qualifications

Education Qualifications

  • Graduate from an accredited and approved school of nursing accepted for licensure in the state of practice is required.

  • Bachelor’s degree in Nursing is preferred.

Experience Qualifications

  • 3 years nursing experience with at least 2 years nursing experience in care management, home health or related experience is required.

Certifications, Licenses and Registrations

  • Licensed as a Registered Nurse (RN) in the state of practice is required. Wisconsin:  RNs may practice with a valid multi-state RN license as outlined in the Nurse Licensure Compact rules.

  • Basic Life Support Certification (BLS) is required within 60 days of hire

Position Details

  • Monday-Friday 7:30am-4:00 pm schedule, must be willing to travel
  • Collaborate with care management teams from Springfield, O'Fallon, Effingham, St. Mary's
